Instructions

To be eligible for documentation of credit, participants must have attended the Cardiometabolic Risk Summit and complete the online evaluation form. After successful completion of the evaluation form, participants may immediately print their documentation of credit. Please follow the below steps:

1. Click General Evaluation or Login at the top right of your screen
2. Login using the email you used to register for the meeting and your temporary password which is: CRS2018. If you registered onsite at the meeting (September 13 or after), your temporary password is not set up and you should choose to create an account. If you already have an existing NACCME account on this site, please use that login email and password.
3. Successfully complete the General Evaluation and ISS Session Evaluations to be eligible for documentation of credit. These evaluations are accessible by using the menu bar located on the right hand side of the page. You can immediately print your documentation of credit. Your certificate will also be accessible for printing at a later date by logging into your account and visiting the “My Account” tab.
   

Please complete your online evaluation by October 16, 2018.
