ABSTRACT

Practice transformation can be daunting to any practice, but imagine if you are solo or small community-based practice in an urban or suburban setting with a large Medicaid patient population? By working with a large number of this type of primary care practice through the Delivery System Reform Incentive Payment (DSRIP) program, making things simple was a key to success in achieving recognition. In this session, attendees will learn techniques and strategies that will assist in a successful transformation.
