ABSTRACT

This session will educate clinic staff on how to implement PCMH, Connected Care or PCSP recognition within their health care systems and what benefits they may receive as a result of the recognition. Thinking beyond what can be offered in the patient-centered medical home and looking at what care the patient needs from their medical neighborhood is where our thinking needs to shift.
