Population Health Meets Integrated Behavioral Health Within an FQHC

ABSTRACT

To better understand and serve patients accessing care at an urban multi-site, multi-lingual FQHC, an interdisciplinary team engages in universal screening for depression, anxiety and substance abuse. This program will explore the rationale behind standardized screening for core behavioral health conditions within a busy primary care clinic, as well as the lessons learned during a large-scale implementation of the program.
