    Partnering with Patients: Gaining Perspective from a Patient Family Advisory Council

    ABSTRACT

    Learning from the voice of the patient is vital to delivering meaningful, collaborative primary care. This presentation focuses on the institution and sustainability of a Patient Family Advisory Council (PFAC) in a small primary care office. We share our experience first-hand through a media-driven time lapse with our practice as we grow through the successes and struggles of practice transformation with our patients and families over the course of 9 months. Join us as we explore partnering with patients to better understand patient motivations, barriers to care, social determinants and their impacts on health outcomes.
